Requirements for Power of Attorney

When a Power of Attorney is submitted for a contract, EquiTrust requires:

1. The Power of Attorney Certification Form (ET-POACERT-2500) to be submitted, in addition to the POA documents and requirements.

2.  If a Power of Attorney is already on file with EquiTrust, we will require the Certification Form to be submitted prior to processing the next service request.

3.  Going forward, while a Power of Attorney may be on file, it is not considered active until the Certification Form is received.

Please note:  If the client does not have POA documents, they can submit the Power of Attorney Appointment and Indemnification Agreement (ET-POAAPPT-2501) to appoint a Power of Attorney to be in effect for Equitrust contracts only.
