When a Trust is named as the Contract Owner, EquiTrust requires:

1. All owner signatures must include "Trustee" after the name

2. First page, plus all pages which address who may act as trustee, and signature page of the Trust

3. Trust Indemnification form signed by the Trustee as Trustee and notarized by an unbiased third party

These documents must be submitted with the application in order to confirm who has authorization to purchase the annuity contract.

Trusts may be Contract Owners for non-qualified annuities only.
