EquiTrust Life Allowable Premium Payments
Effective June 20, 2008

The only acceptable forms of payment will be:

• Checks payable only to the Company from the contract owner.
• Transfer from trust accounts.
• Payments from insurance companies whose policies/contracts are exchanged for a contract issued by the Company.
• Transfers from one financial institution to another.
• Cashier's checks from a financial institution.*
  
  The following forms of payment will not be accepted:

• Currency (domestic or foreign)
• Traveler’s Checks and credit card checks
• Money orders
• Cashier’s Checks from places other than financial institutions
• Other cash equivalents
• Wire transfers directly from a foreign financial center
• Checks drawn from distributor accounts (other than for contracts for members of the agent’s immediate family).

* Cashier's checks include those labeled as "treasurer's checks" or "bank checks."
