EquiTrust contracting documents are revised, and now include a new form required by Vector One (commission debit-balance screening service): Agent Debit Check Authorization (ET-DEBITCHECK).
Update All Contracting Forms Now The revised contracting forms – dated 11-20 – may be used immediately, and are available on the Agent Gateway website: Agents.EquiTrust.com. The revised forms will be required beginning December 1, 2020.
