Effective immediately, the Trust Information Form and Trustee Certification and Indemnification Agreement – ET-TRUST-2502(4-16) – has been revised and is available on the Agent Website: Agents.EquiTrust.com. Click here to download the revised form.

EquiTrust will no longer require copies of any pages of the trust with respect to new business or inforce contracts. EquiTrust will now only require the updated Trust information and Trustee Certification and Indemnification Agreement be completed and submitted. With respect to death claims, current EquiTrust procedures remain unchanged.
