 Revised Agent Appointment Application Required July 1, 2021
The Agent Appointment Application has been revised. The new version ET-3200(05-21) is available now on the Agent Gateway Website and may be used immediately. It will be required for agent contracting paperwork received on or after July 1, 2021.
Agents writing business for the first time in over 24 months need to submit the 5-21 application and the Vector Debit Balance Authorization.
