FL Suitability/Replacement Forms
10-Day Rule Enforced
As was announced previously, the state of Florida began requiring a Florida-specific suitability form (form number DFS-H1-1980) and a Florida replacement comparison form (form number DFS-H1-1981) (if applicable) with each annuity application.  This requirement went into effect October 1, 2013.
10-Day Rule Enforced
The state of Florida also requires that these forms are to be received by the  annuity issuer within 10 calendar days of the execution of the forms. EquiTrust will require that such forms are received in our office within 10 days of the date they are signed by the applicant. 
 
Effective June 13, if applications are not received within 10 days of the date signed, we will contact the agent to have these forms signed and dated again, then returned to EquiTrust.
