New Service Forms – Available Now; Required 1/1/20

Multi-purpose service forms replaced by specific-need forms

The former Service Request Forms (ET-2514 and ETL-2514) were used to request a variety of changes to contracts and policies, including name, address, owner, IRA conversion, and duplicate policy/contract requests.  This form has been retired, and replaced by specific-need forms. 

The new forms may be used immediately.  The old forms are no longer available on the website, and will be accepted through 12/31/19.
