Here are some helpful reminders to make sure the process goes smoothly:
We require an original, certified copy of the death certificate, with cause of death included, for death benefits greater than or equal to $250,000.
A photocopy of the certified death certificate, with cause of death included, is acceptable for claims less than $250,000.
If a beneficiary predeceases the owner/annuitant, we require a photocopy of their certified death certificate.
